Health Assessment

Regular health assessments give you the opportunity to catch potential health issues early, maintain your wellbeing and plan for a healthier future. Whether you're looking to establish baseline health markers, manage existing conditions or simply want peace of mind about your health status, comprehensive health assessments are an investment in your long-term quality of life.

Patient Eligibility

Below are groups of patients eligible for government-funded health assessments:

  • People aged between 45 and 49 who are at risk of developing a chronic condition

  • People aged 75 years and older

  • People aged between 40-49 years at risk of developing type 2 diabetes

  • People living with an intellectual disability

  • People who are a refugee or humanitarian entrant

  • People who are veterans (one-off)

What will my GP do?

Prevention is key to overall health. Health assessments allow us to identify potential problems before they develop. Health assessments involve working with both our practice nurse and GP. All health assessments are tailored to each patient's requirements; however, all health assessments will include the following:

  • Information collection - taking comprehensive patient history

  • Examination - undertaking or arranging examinations and investigations as required

  • Overall assessment - making an assessment of the patient's condition

  • Interventions - recommending appropriate interventions and referrals

  • Patient advice - providing advice and information to the patient

How long will it take?

Typically, it takes between 30 and 60 minutes to undertake a health assessment. This time is split between a consultation with a practice nurse and a consultation with your GP.
