Chronic Condition Management

A Chronic Condition Management Plan, commonly referred to as a care plan, is a personalised health plan created by your GP to help you manage a long-term medical condition, such as diabetes, asthma, or heart disease. It outlines your health goals, treatments, and the support you need, and can also involve other health professionals like physiotherapist or dietitian. This plan ensures your care is well-organised and tailored to your needs, and may give you access to Medicare rebates for certain allied health services.

Patient Eligibility

If you’ve been living with a medical condition that has lasted, or is likely to last, six months or more, you may be eligible for a Chronic Condition Management Plan (CCMP), commonly referred to as a care plan. Common long-term conditions include diabetes, asthma, arthritis, and heart disease.

What will my GP do?

Managing a long-term health condition can be challenging, but you don’t have to do it alone. A Chronic Condition Management Plan is designed to help you and your GP take control of your health, with regular support and check-ins. Our practice nurses will work with you and your doctor to create a plan that suits your needs and helps you stay on track.

In addition to formalising the management of a chronic condition, having a Chronic Condition Management Plan provides funding for up to five allied health visits per calendar year.

How long will it take?

Typically, it takes around 45 minutes to create a Chronic Disease Management Plan. This time is split between a consultation with a practice nurse and a consultation with your GP.
